Case Report Request Form

Case reports will be sent upon receipt of an open records request.  This web form will serve as that request.  While submitting the date of the incident is essential, providing information in the additional fields will help to expedite your request as there can be more than one occurrence on the same date.

There is a charge of $0.25/page for any report over 8 pages in length.  Please include a phone number so that we can contact you regarding payment.  Please be advised that there may be charges for sending reports via US Mail and emailing allows us to be more flexible and efficient with your request.  We will make contact with you for costs related to extensive records requests.
